What to Expect at Your Initial Evaluation

I’m looking forward to meeting you!

Your 45-minute Initial Physical Therapy Evaluation is an opportunity for us to take a closer look at what you’ve been experiencing and determine the best path forward.

During your appointment, we will:

Talk through your symptoms, health history, and concerns
Assess your movement, strength, mobility, and other areas related to your symptoms
Discuss your goals and what you would like to get back to doing
Review my findings and recommendations
Discuss a personalized plan of care if additional physical therapy is recommended

A Few Things to Help You Prepare

Complete your forms: Please complete your New Patient Intake & Consent Form before your appointment:

Wear comfortable clothing: Choose clothing that allows you to move easily and provides access to the area we will be evaluating when appropriate.

Bring relevant information: If you have recent imaging reports, physician recommendations, or other information related to your current condition, feel free to bring them with you.
